This is a patient referred after an aborted attempt at Yamane ISHF where the iris was damaged and the IOL dropped posteriorly. There was an iridodialysis repair done at the time of the initial surgery with exposed 10-0 prolene suture causing irritation at 12:00. The broken off haptic from the previous IOL is also extruding in this area and there is cornea edema. The surgical plan here is to retrieve the IOL, remove and replace with a new Yamane ISHF lens, fix the iris, do DSAEK and patch the leaking area at 12:00 with cornea tissue (the stroma from DSAEK donor rim).
